How to Prepare
We like to have everything organized and ready before you even step through our front door. To help us prepare for your visit, please complete a few quick steps before your appointment:
- Log in to your Patient Portal. (Don’t worry—we’ll send a link straight to your inbox. It’s that easy!)
- Fill out your New Patient Paperwork and Health History Forms.
- If you have insurance, make sure we have your up-to-date information at least a week before your appointment.
- If you’re a new patient, please arrive 15 minutes early so we have plenty of time to discuss your questions, concerns, and hopes for your smile.

We accept many dental insurance plans and offer a variety of payment options to work within your budget.
